Any changes to the time and space occupied after approval must be requested, using the Minor Amendment button that appears once your application is approved. This includes early completions! Photos are to be uploaded before and after occupation, and in some cases during, using the Upload Site Photo button that appears once your application is approved. See Conditions for the specifics.

If you have not been provided the required information for this section by your City Project Manager, please contact your City Project Manager to provide you with the percentage breakdown for each “asset owner/ client” before completing this section. The accuracy of this information is important as it will be used for invoicing purposes.
 
If your project is bundled ,please add a new row by clicking the “Add New Row” button or each asset owner/ client associated with your project. Each row must be assigned a percentage, and the total of all rows must be 100%.
 
If your project is not bundled, select your asset owner/ client from the drop‑down menu and enter 100% in the percentage allotment section.
 
If the asset owner/ client is not listed in the drop‑down menu, please select “Other” and provide the legal, official name of the asset owner in the “ If Other, please provide details” field below and assigned a percentage accordingly.

Include roadway occupancy anomalies that may need to be noted, compliance issues specific to traffic standards such as the OTM, Book 7, 2022 edition, turn restrictions, and or paid duty officers. If additional sketches are uploaded, note them here and upload using the Other Files feature below.

Traffic plans must present the actual environment, include all key measurements, and use OTM Book 7, version 2022, references. They must also satisfy AODA requirements. For more specifics, review the Conditions for this application. Please clearly identify the location for each TMP submitted.
